    JOB SUMMARY

    The Care Navigator LVN provides direct nursing care to an assigned group of patients, under the appropriate supervision of a Registered Nurse, APRN , by applying clinical nursing knowledge, skills, and ensuring the safety and comfort of patients and families according to legal, organizational and professional standards. The Care Navigator LVN prioritizes and makes judgments concerning basic needs of multiple patients in order to organize care. Assists with outreach initiatives and goals set forth by the Population Health leadership. Ongoing liaison between patient, multispecialty care departments and approved Facilities.

    EDUCATION REQUIREMENTS & EXPERIENCE REQUIREMENTS

    Graduate of an accredited LVN program..

    Experience

    A.

    3-5 years’ experience in prior nursing experience in an outpatient or acute care setting.

    Care Management / Disease Management experience

    Licenses

    A.

    Licensed Vocation Nurse in the State of Texas

    B.

    Special Skills

    A.

    Current BCLS

    B.

    Other

    A.

    Technology proficiency (use of EMR, excellent computer skills) ability to interpret data via reports

    EPIC experience; Windows based computer skills
