Overall Job Objective

Position Description

Under the supervision of the Health Impact Team (HIT) Manager, the Mobile Health LVN is a member of the community mobile Health Impact Team (HIT). The Mobile LVN is responsible for supporting the clinical program of the Harris County Mobile Expansion Health Program, which serves patients across the four precincts of unincorporated Harris County.

Job Duties & Responsibilities
Mobile Clinic Activities

  • Performs mobile direct care in the community, health centers, health hub, mobile villages in accordance with regulations of the Texas Board of Vocational Nurse Examiners
  • Represents Harris County Wellness Clinic and the mobile unit on community committees, and at outreach and community events
  • Interviews clients and records medical/psychosocial information in electronic medical records
  • Assists with patient appointments by performing phlebotomy (including capillary and venous draws), urine collection, pregnancy tests, lead testing, vital signs, patient histories, etc.
  • Participates in performing quality assurance and checks on lab specimens and preparing specimens for transport
  • Administers/reads tuberculosis skin tests as needed
  • Assists the Midlevel Providers, Physicians, and Medical Director in tracking and monitoring of abnormal lab results and reschedules lapsed appointments to the Mobile Medical Health Impact Team
  • Dispenses prescribed medication according to Pharmacy D regulations and HCPH procedures and protocols
  • Attend mobile unit clinics as needed to train staff, observe and fine-tune workflows to ensure fulfillment of organizational priorities
  • Assists in preparing statistical reports
  • Maintains confidentiality of client's information
  • Maintains knowledge of CDC recommended vaccines and administers age appropriate immunizations as required
  • Enhances and encourages creativity to continue to develop a cutting edge community outreach plan

Administrative Duties
  • Assists with mobile inventory and replenishes needed supplies on the mobile unit, and arranges supplies in order of expiration
  • Acts as an alternate immunization coordinator for the Department of State Health Services Vaccine For Children Program
  • Expands and strengthens referring provider network

Requirements

  • Graduation from an accredited school of Vocational Nursing
  • Current license to practice as a Vocational Nurse issued by the State of Texas Board of Nursing
  • Basic knowledge of Microsoft Office suite and ability to navigate through electronic medical records
  • Valid driver's license (Texas upon hire) and reliable transportation for daily travel to various locations and clinics as assigned

Preferences

  • Certification in phlebotomy or blood withdrawal
  • Experience working in a community health or mobile environment
  • Experience with Family Planning, Title X, and Healthy Texas Women grant
  • One (1) year of experience as an LVN in a public health or community health setting
  • Experience working with EPIC
  • Bilingual in English and Spanish, Arabic, Vietnamese or other language

General Information

 HOURS:

  • 40 Hours per Week
  • Standard Monday-to-Friday schedule
  • Subject to Evening and Weekend hours as required based on mobile services schedule

SALARY:     
  • To Be Discussed During Departmental Interview
  • Paid biweekly based on 26 pay periods
