Job: MDS/Care Plan Coordinator

Celebrate Caring at Lexington Place Health Care Center, an affilated center of Preferred Health Care. We are currently seeking a full time MDS/Care Plan Coordinator at our 200-bed skilled nursing center located in Houston, TX.

Position Summary:
Assist in assessing and Care Planning each residents condition annually, quarterly or when significant change in condition occurs utilizing the RAI process.

Responsibilities include but not limited to the following:

The coordination of patient assessments and care planning to provide the highest quality of care to the patients.

Overseeing the assessment process.

Preparing assessment schedules and ensures that assessments and care plans are done accurately, completely and timely.

Tracking acknowledgment after electronic submissions.

Keeping abreast of MDS and PPS changes and updates.

Providing in-services to the interdisciplinary team on updated MDS information.

Communicates and works closely with the interdisciplinary team.

Acts as the decision maker in case of a disagreement with an assessment or an item pertaining to the MDS or care plan.

Completing Initial, Quarterly, Annual, and Significant change MDS assessments

A candidate for this position should be:

We are looking for a graduate of an accredited School of Nursing (RN or LVN) with a valid state nursing license and CPR certification

Accurate, concise, detail-oriented and able to work cooperatively as a member of a team.

The ability to organize, prioritize and communicate effectively with residents, their families and all levels of the organization.

Must have Long Term Care experience

Must have the knowledge to obtain high rug scores

Should be able to react decisively and quickly in emergency situations and maintain confidentiality.
