Job: Registered Nurse----PRN/On-Call

Admit clients to our services, completing appropriate documentation. Assess individual status and care/instruction needed by the patient.

Direct on-going care and provide necessary follow-up.

Confer with clients, family members and develop the initial plan of treatment based on physicians orders and initial patient assessment.

Revise plan as necessary, based on ongoing assessments and as required by policy/regulation.

Coordinate appropriate care, encompassing various healthcare personnel (such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.)

Report patient care/condition/progress to patients physician and Home Care Manager as needed. Analyze any changes in a clients condition, identifying symptoms and severity while alerting the physician and Home Care Team promptly.

Implement patient care plan, in conjunction with patient and family, to assist them in achieving optimal resolution of needs/problems.

Qualifications

Associate or Baccalaureate Degree in Nursing

Highly motivated individual

Current unrestricted licensure to practice as a Registered Nurse in Texas.

Good organizational and communication skills

Ability to function with minimal supervision

Available for On-Call every other weekend

This is a PRN/On-Call position, great for students or the semi-retired nurse.
