Currently we are seeking a RN Home Health Nurse to join our team!

Home Health Resources believes in continuity of care! We believe that the BEST patient care is delivered by clinicians who know the patients intimately, and to that end, all of our nurses continue to see their patients and perform all nursing visits until the patient is discharged from services.

Responsibilities include:

Admission Assessments including OASIS

Routine Nursing Visits as ordered by the Physician on the Plan of Care

Documentation of all care and coordination utilizing a computerized documentation system/ handheld computer

Communicate and coordinate care with the patient, family, physician, and other disciplines in the home to ensure all needs are addressed.

Must be a Registered Nurse in good standing in the State of Texas.

One year experience as a professional nurse required in an acute care setting required; home health experience a plus.
We'll train the right person without home health experience.

Must be able to read, write, and communicate clearly in English.

Knowledge of Medicare rules and regulations a plus.

Computer literate preferred, as we use a point of care documentation system.
